actually it's happened quite a few times, when the IESG didn't get their 
paperwork done in time - everyone agreed that the group should be 
chartered, and the charter was basically fine, modulo a few tweaks, but the 
tweaks just didn't get done in time for an IETF meeting.

Then the WG-to-be formally meets as a BOF.
